On November 8, 2010, we received a report that a personal laptop computer containing information for a specific group of patients was stolen from a Dean Clinic physician in a home invasion robbery. The theft was reported to law enforcement the same day and immediately investigated.
Patients who may have been affected are being contacted via letters mailed on December 18, 2010.
Through our own internal investigation, we learned the laptop contained limited amounts of protected health information including patient names, dates of birth, medical record numbers, diagnoses, procedures and possibly pathology data. We can assure you that the laptop did not contain Social Security numbers, credit card information, home addresses, phone numbers or any financial information.
We are sincerely sorry this happened and want to provide you with pertinent information along with the steps we are taking to minimize any potential impact.
